Medical Records Request

Please utilize the form below when requesting medical records and email to info@horntherapy.com or fax to (859)224-4082. Per Kentucky statutes and in accordance with Centers for Medicaid and Medicare Services regulations, medical records are maintained for 6 years from date of discharge or, in the case of records for a minor, records are kept until the client is 21 years of age (or the 6 year period if that is longer). Records that extend beyond the above time frame will not be available. Thank you!
